During this time, you can still apply to another lab, but you may not receive an approval response from the state. If after a period of 30 days you are still not notified, contact the CDP: 1-855-5-LAB Testing Services (during office hours) at 1-855-5-LAB Testing Services (outside of office hours). The CDP will not review applications that are received after this 30-day period. Application Fee (25.00): Payment to the CDP's Customer Service call-in number is needed. A nonrefundable 25.00 fee is required to apply to the CDP, unless otherwise specified on the CDP application. If you wish to make an extension of the application fee, you must contact Customer Service at 1-855-5-LAB Testing Services.
You must pay this fee before you can process the application. If you have applied the fee before, you may not reapply, and you cannot request a refund. If you submit an application after the deadline for the current semester and the deadline to notify the state is passed, another application fee will be charged.
